Parents can now log on to our secure system and print electronically certified copies of DH 680 Form if their health care provider is a participating physician. Electronically certified shot records are available for quick access with a registry-issued ID and pin number.
Schools and day cares accept electronically certified forms (authorized by Rule 64D-3.046 Florida Administrative Code).

New Students in Florida Schools
If your child is a new student in Escambia County, out-of-state immunization records will need to be transferred to a DH 680 form.
