Health starts long before illness; it begins in our homes, our schools, and our jobs. Through data analysis, surveys of the community, and focus group discussions health priorities are picked. These health priorities will be the focus of our work for the next three years. Together our community identifies goals, strategies, and objectives for these health priorities. This process involves community focus groups and data analysis to identify areas of strengths, weaknesses, opportunities, and threats regarding improving population health. 

The Community Health Improvement Plan (CHIP) is a way to document, monitor, and evaluate the community’s progress towards meeting set goals and objectives. The objectives noted in this plan are monitored over the three-year period. Annually, the CHIP is reviewed, and objective status is updated to indicate progress being made (or not).
