Chronic Disease Self-Management (CDSM) Workshops

April is Minority Health Month, and the Florida theme is “Healthy Communities Build a Healthier Florida.” The Florida Department of Health, Escambia County, in collaboration with Florida’s Office of Minority Health, invites you to an informational session about implementing the Chronic Disease Self-Management (CDSM) Workshops in our community.

 The CDSM is an evidence-based chronic disease self-management program developed by Stanford University and partners through their Patient Education Research Center. This six-session program aids in assisting clients who have chronic health conditions to properly manage their health and improve their quality of life through simple lifestyle changes in a supportive group environment. The target populations for this program are people with one or more chronic health conditions like hypertension, diabetes, heart disease, and cancer who may be noncompliant with their medication usage or general health management. The self-management toolbox will cover healthy eating, being active, medications, decision-making, problem-solving, communication, action planning and more.

  • This webinar will be provided in English in partnership with Center for Change (CTG Provider). This webinar intends to inform Department staff, Community Health Providers, Local Partners, and CHWs about the importance of educating and empowering communities to self-manage chronic conditions and how to initiate Chronic Disease Self-Management Program (CDSMP) Workshops in their local community with the goal of improving the number of trained CDSMP instructors throughout the state.

  • This webinar will be provided in Spanish in partnership with the Caridad Center (CTG Provider). This webinar intends to inform Department staff, Community Health Providers, Local Partners, and CHWs about the importance of educating and empowering communities to self-manage chronic conditions and how to initiate Chronic Disease Self-Management Program (CDSMP) Workshops in their local community with the goal of improving the number of trained CDSMP instructors throughout the state.
