Florida birth certificates are issued through the Bureau of Vital Statistics at the Department of Health. Orders may be placed online, by mail, or in-person.

Please provide:
- A valid (non-expired) form of photo identification
- Driver’s license
- State-issued ID
- Passport
- Military ID
- A completed Birth Certificate Application Form
- Please note: the application form must include the person’s full name, date of birth, county of birth, mother’s full name (including maiden name), and father’s full name.
Birth certificates can be ordered at the following locations:
- Fairfield Service Center – Monday through Friday from 8 a.m. to 4:30 p.m.
- Downtown Service Center – Monday through Friday from 8 a.m. to 4:30 p.m.
- Northside Service Center – Tuesday through Thursday 9 a.m. to 4 p.m.

Birth certificates issued prior to 1930 and birth records that are under seal must be requested from the Florida Department of Health’s Bureau of Vital Statistics in Jacksonville.
