Birth Certificates are often needed for entrance to school, voter registration, marriage, passports, veteran benefits, public assistance, social security benefits and obtaining a driver’s license.
Florida birth certificates are issued through the Vital Statistics Office at the Florida Department of Health in Escambia County (DOH-Escambia). Orders may be placed in person, by mail, or online through VitalChek.

A valid photo ID (driver’s license, state ID card, passport, or military ID) is required with all orders. Birth certificate fees are $11.00 for the first copy; $10.00 for each additional copy. Forms of payment accepted are: cash, money order, debit card, Visa and MasterCard. Please make money orders payable to DOH-Escambia.
Florida Birth Certificates Can Be Obtained By:
- Either parent (if named on the certificate)
- Registrant, if over 18
- Attorney representing the registrant or parent (if application made on letterhead)
- Legal guardian with guardianship papers
Application for a Florida Birth Certificate MUST Include:
- Full name of the registrant as it appears on the certificate
- Date of birth
- City or county of birth
- Names of both parents (including mother’s maiden name)
- Signature of applicant
- Relationship to registrant
- Mailing address
- A copy of a valid photo identification (front and back)
- The fee ($11.00 for 1 copy; $10.00 for each additional copy).
Amendments to a Florida Birth Certificate:
If there is an error on your or your child’s Florida birth certificate, contact the State Office of Vital Statistics in Jacksonville, Florida at 904-359-6900, Extension: 9005. Amendment forms are available on the FloridaHealth Certificates and Registries web page or can be picked up at DOH-Escambia. This form must be notarized.If the Person was not Born in Florida:
