FLORIDA DEPARTMENT OF HEALTH IN ESCAMBIA COUNTY ANNOUNCES TEMPORARY CLOSURE OF COVID-19 VACCINATION ONLINE INTEREST FORM

By FDOH-Escambia

January 15, 2021

Pensacola, Fla. – The Florida Department of Health in Escambia County (FDOH-Escambia) is announcing the temporary closure of its COVID-19 Vaccination Request Form effective today, January 15, 2021.

FDOH-Escambia continues to work through the list of individuals who previously completed the online form. As supply becomes available, FDOH-Escambia staff will be scheduling appointments. We also encourage residents to take advantage of opportunities offered through other partners to get vaccinated.

Additionally, FDOH-Escambia is working to identify and implement a more efficient solution to gauge interest in, and schedule appointments, for vaccine. If you have questions, please call FDOH-Escambia’s COVID-19 phone line at (850) 595-6500 option 6.

The COVID-19 vaccine is a monumental step toward getting this disease under control in Florida, but the department is still encouraging residents and visitors to take all precautions to protect themselves and others from the spread of the virus. These include frequent hand washing, wearing face masks, maintaining six feet of social distancing, and staying home if you are feeling sick.
